OUTLINE OF WELFARE FUND BENEFITS
I. Supplements to Basic New York City Health Plans for ACTIVE EMPLOYEES
- GHI-CBP
- NPA Prescription Drug Card (and Central Fill mail order plan) in place of GHI drug rider
- Welfare Fund Major Medical supplement ($2,000 annual deductible $1,000 if you buy the city rider).
- HIP/HMO and HIP CHOICE
- HIP optional rider
- Prescription Drugs, Appliances and Private Duty Nursing
- Additional mental health benefits, covers $25 co-pay per visit for HIP/HMO and HIP Choice Plus
- PHS, Choice Care, Blue Choice
- NPA Prescription Drug Card (and Central Fill mail order plan)
- All other health plans (HMO'S)
- Drug rider for that health plan

- This benefit is available only to eligible active employees and their dependents who are enrolled in the GHI/CBP Blue Cross program. Eligible members will have a $1000 out-of-pocket expense before being reimbursed.

- Death Benefit - $10,000. This benefit is for the member only.
- Accidental Death, Dismemberment and Loss of Sight Benefit - $10,000
- Podiatry Benefit
- The Podiatry Benefit is provided at the DC 37 Health Center at 115 Chambers Street in Lower Manhattan (212) 766-4455 and 186 Joralemon Street in downtown Brooklyn (718) 625-2544, available to members only.
III. Audiology and Hearing Aid Benefit
- The Audiology benefit is provided at the DC 37 Health and Security Plan Audiology Center only, which is located at 125 Barclay Street (Room 770), New York, NY, (212) 341-4790. The benefit is available to members only.
- Supplement to the GHI/CBP (Catastrophic Medical benefits)
